Care Plans
By
performing a thorough needs analysis we create a
customized Care Plan that is specifically developed
based on the needs of your loved one. Depending on
needs, we may utilize either a registered nurse or a
professional geriatric care manager to perform the
initial assessment and to develop the initial Care
Plan. This assures that the right type of professional
is involved from the very start. By using this process,
we ensure that each client’s needs are understood and
that the assigned Caregiver’s skills best match those
needs.
Should you
desire, we will ask the Caregiver to meet with you and
your loved one prior to starting service to make sure
there is a match.
