Care Plan
Review
Throughout
the duration of the care period we perform periodic Care
Plan reviews. We complete these updates to help ensure
that the care services being performed stay up to date
with the needs of your loved ones. Reviews help to
ensure the satisfaction of both the client and the
family. Our review process includes the management team
and the Caregiver and, depending on the complexity of
the case, the reviews may include a nurse or geriatric
case manager. Should a change in the Care Plan be
considered, we will communicate our recommendations to
the client or their family.
Our
management team will communicate regularly to confirm
that all parties are satisfied with the care and
services that are being received. If there are any
concerns, we will work diligently to solve any issues
before they become problems.
